Streamline Automation & Systems (“SAS”) respects your privacy. This policy explains what information we collect through this website and how we use it.

Information we collect

When you submit the Business Modernization Scorecard, we may collect your business name, contact name, email address, optional phone number and website, scorecard answers and scores, biggest challenge, additional notes, landing page URL, referring page, and campaign attribution fields.

How we use information

SAS uses this information to receive and personally review your scorecard, prepare or discuss your Business Health Report, respond to your request, improve our services, and maintain reasonable business records.

Service providers

Form delivery and website operation may use trusted service providers that process information on our behalf. For example, FormSubmit may be used to deliver completed scorecards by email. We do not add invasive tracking cookies or third-party advertising trackers through this lead system.

Sharing and retention

We do not sell your personal information. Information may be shared with service providers as needed to operate the website and deliver your submission, or when required by law. We retain information only as reasonably necessary for the purposes described above.

Your choices

You may request access to, correction of, or deletion of information you submitted by contacting SAS. Some information may be retained when required for legal, security, or legitimate business-record purposes.
